Weight-adjusted-waist index mediates the association between Life's Crucial 9 and sarcopenia

Introduction:
Background: research has shown a likely link among cardiovascular health, obesity, and sarcopenia. Nevertheless, the mechanisms by which lipid metabolism affects the connection between cardiovascular health and sarcopenia are not fully understood. Life's Crucial Nine (LC9) is a recently proposed method for assessing cardiovascular risk. Concurrently, the weight-adjusted waist index (WWI) is a novel and more precise indicator of obesity. This research investigates the interplay between LC9 and sarcopenia and assesses whether WWI is a mediator in this association. Methods: various methodologies were utilized, including multivariate logistic regression, smooth curve fitting, and subgroup analysis. Moreover, a mediation analysis was performed to investigate the potential interaction between WWI levels and LC9 about sarcopenia. Results: this examination involved 7,780 subjects, among which 644 indicated that they had encountered sarcopenia events. Upon adjusting for all covariates through multivariate logistic regression analysis, we discovered that a 10-point rise in the LC9 score correlated with a 36 % decrease in the prevalence of sarcopenia (Odds Ratio, OR = 0.64, 95 % CI: 0.60, 0.68). In contrast, an increase of 1 unit in the WWI score was associated with a 9.08-fold rise in the sarcopenia risk (OR = 9.08, 95 % Confidence Interval, CI: 7.27, 11.35). Moreover, consistent findings emerged when LC9 and WWI were analyzed in tertile. The analysis of smooth curve fitting demonstrated a linear relationship among LC9, WWI, and the probability of sarcopenia. Additionally, the findings from the mediation analysis indicated that WWI mediated 88.14 % of the relationship between LC9 and sarcopenia (p < 0.001). Conclusion: the prevalence rate of sarcopenia at the LC9 level demonstrated a noteworthy inverse relationship, with the weight-adjusted waist index acting as a partial mediator in this association.
