Abstract:
Obsessive-compulsive disorder (OCD) and posttraumatic stress disorder (PTSD) frequently co-occur; however, little is known about veterans' experiences with this comorbidity. This study explored veterans' experiences with OCD and PTSD symptoms and mental health treatment at Veterans Affairs (VA), as well as current treatment barriers and needs to enhance VA care. Veterans with documented diagnoses of OCD and PTSD were identified through VA medical records. Semistructured interviews were in part informed by the updated Consolidated Framework for Implementation Research. Interviews were analyzed using rapid, matrix-based qualitative analysis methods. Eleven veterans (majority Black/African American and female, ages 38-71) participated. Veterans largely believed their OCD and PTSD symptoms were related (i.e., dynamic comorbidity) and most often received prolonged exposure therapy for PTSD. Perceived barriers to treatment for this comorbidity included fears of stigma related to OCD, lack of provider understanding and focus on OCD, and logistical challenges. Perceived needs included improving provider understanding and focus on OCD, reducing stigma, and increasing access to information about OCD in VA. Findings highlight the need for system-wide initiatives to improve awareness, reduce stigma, and enhance the focus on OCD within VA. (PsycInfo Database Record (c) 2026 APA, all rights reserved).
