Long-term hearing outcomes and prognostic factors in Meniere's disease

Background:
While the management of vertigo in Meniere's disease (MD) has been extensively studied, there is limited understanding of the long-term hearing prognosis and associated risk factors.
Aims/Objectives:
To assess the long-term hearing outcomes and identify key factors influencing hearing prognosis in patients with MD.
Materials And Methods:
A retrospective review was conducted on 159 patients (179 affected ears) with MD, with a minimum follow-up of six months. Hearing outcomes were classified based on four-tone (500, 1000, 2000, and 3000 Hz) averages (FTA) into stages. Univariate and multivariate analyses were performed to determine factors associated with post-treatment hearing stages.
Results:
The mean follow-up duration was 4.7 years. At baseline, 36% of patients had stage I/II hearing, whereas 64% had stage III/IV. Post-treatment, 33% remained at stage I/II, and 67% at stage III/IV. Multivariate analysis revealed two independent predictors of better hearing outcomes (stage I/II): younger age (<58 years) (OR 6.28, p < 0.001) and better pretreatment FTA (<50.0 dB) (OR 28.52, p < 0.001).
Conclusions:
Long-term hearing outcomes in MD are primarily influenced by age and baseline hearing status. Early intervention and treatment in patients with preserved hearing may improve prognosis. These insights can assist clinicians in risk stratification and tailoring individualized management strategies.
