Background:

Digital health technologies (DHTs) require robust assessment frameworks.
Existing digital health technology assessment (DHTA) guidelines exhibit international variation.
Standardization of DHTA is crucial for global policy development.

Purpose of the Study:

To systematically identify and compare global official DHTA guidelines.
To analyze methodological quality, domain coverage, and cross-national patterns.
To inform international standardization of DHTA.

Main Methods:

Systematic review adhering to PRISMA 2020 guidelines.
Searched PubMed and grey literature for official DHTA guidelines (2019-2025).
Assessed quality using AGREE II and coverage using EUnetHTA Core Model.

Main Results:

Twelve guidelines from 10 countries were analyzed, showing moderate average quality (65% AGREE II score) with significant variation.
Guidelines covered 61% of EUnetHTA domains on average, with high inclusion of clinical effectiveness and safety, but lower inclusion of legal and ethical aspects.
Substantial cross-national heterogeneity was observed, especially in Rigor of Development and Stakeholder Involvement.

Conclusions:

Official DHTA guidelines are increasingly established globally but display significant heterogeneity, particularly in governance.
Enhanced methodological rigor and international alignment are necessary for consistent and impactful DHTA frameworks.
Further standardization can improve the policy relevance of DHTA globally.
