Background:

  • Chronic radiation proctitis (CRP) is a long-term complication of pelvic radiotherapy for locally advanced rectal cancer (LARC).
  • Understanding the endoscopic natural history and predictors of severe CRP is crucial for patient management.

Purpose of the Study:

  • To characterize the long-term endoscopic natural history of CRP after neoadjuvant therapy for LARC.
  • To identify predictors of severe and persistent CRP lesions.

Main Methods:

  • Secondary analysis of the STELLAR study involving 74 LARC patients undergoing anal-preserving surgery.
  • Systematic endoscopic follow-up (>2 years) with Vienna Rectoscopy Score (VRS) grading.
  • Multivariate analyses to identify risk factors for severe (VRS≥3) and persistent severe CRP.

Main Results:

  • Hypofractionated radiotherapy (TNT) showed a trend towards higher incidence of severe CRP (33.3% vs 20.0%, P=0.197) but a favorable recovery pattern compared to long-course chemoradiotherapy (CRT).
  • Endoscopic severity peaked at 2 years post-radiation, with recovery noted in the TNT group but not CRT.
  • Diabetes mellitus (HR 5.46) and ECOG performance status 1 (HR 8.81) were independent predictors of severe and persistent CRP.

Conclusions:

  • Hypofractionated radiotherapy (TNT) does not increase severe CRP risk and demonstrates a better recovery profile.
  • Diabetes and reduced performance status are significant predictors of severe and persistent endoscopic injury.
  • Findings support tailored surveillance and preventive strategies for high-risk LARC patients.
