Surface-based morphometry in lesioned brains after virtual brain grafting: CAT12 versus FreeSurfer in low-grade

Background:
The cortical thickness (CT) measurements are important for research of structural neuroplasticity in low-grade gliomas (LGG). Virtual-brain grafting (VBG) is the novel method of pre-processing magnetic resonance imaging (MRI) scans of lesioned brains. Originally, FreeSurfer is engaged in the workflow. Computational Anatomy Toolbox (CAT12) gives CT values at lower computational cost, in shorter time, and is easier to use. Until now no study determined the agreement between CAT12 and FreeSurfer in VBG pre-processed brains.
New Method:
We included MRIs of 35 LGG patients and 31 healthy controls (HCs). Each lesioned MRI underwent VBG pipeline. MRIs entered CAT12 workflow and, for comparison, FreeSurfer workflow. We collected CT values according to Desikan-Killiany atlas. We used coefficient of determination (R²), Bland-Altman (BA) plots, linear mixed model (LMM), and intraclass correlation coefficients (ICC) for outcome comparison.
Results:
We found R² = 0.65 for HCs, and BA plots showed higher values of CT for CAT12, with a difference of 0.064 mm. For patients, CAT12 also gave higher CT values, with lower correlation of R² = 0.40 and larger mean difference of 0.1675 mm. This indicated lower consistency for patients. The values of ICC were generally lower in patients.
Comparison Of Existing Methods:
LMM indicated that the agreement between CAT12, and FreeSurfer originally implemented in the pipeline, was associated with the presence of tumour, ROI, and the volume of tumours.
Conclusions:
The agreement between FreeSurfer and CAT12 in VBG pre-processed brains of patients with LGGs is lower than in healthy brains.
