Variation in time to payment in Medicaid managed care: evidence from national TAF data, 2022

Timely payment of Medicaid claims is critical for provider cash flow, administrative burden, and participation in managed care networks. Although federal and state prompt-pay standards apply to Medicaid managed care organizations (MCOs), national evidence on payment timeliness remains limited.

Using 2022 national Transformed Medicaid Statistical Information System (TAF) claims data, we examined time to payment for professional and ancillary services inclusive of time associated with provider billing and subsequent payer reimbursement, billed to Medicaid MCOs in 39 states and the District of Columbia. We characterized claim-level payment distributions, service-specific patterns, and variation across MCOs and states. We estimated state and MCO fixed-effects models to assess the share of variation attributable to plan- vs state-level differences.

Median time to payment was 26 days (interquartile range, 32 days), with 90% of all professional claims paid within 128 days. Payment times were similar across most selected service types but varied across MCOs and states. While plans paid most claims within 30 days, 53 MCOs paid more than 25% of claims at or beyond 90 days, and 8 MCOs paid more than half of claims after 90 days.

Time to payment can be substantial and in some cases, takes several months, with important implications for provider participation, oversight, and network stability.
