Mapping Oxygen Saturation and Its Correlation With Motility in Diabetic Enteropathy: A Photoacoustic Imaging Study in

Background:
Diabetic enteropathy (DE), a common complication of diabetes, is characterized by intestinal dysfunction such as diarrhea and constipation. This study aimed to quantitatively assess changes in intestinal oxygen saturation and motility, and to delineate their correlation, in streptozotocin-induced type 1 diabetes mellitus (T1DM) mice using photoacoustic imaging (PAI) and speckle tracking imaging (STI).
Methods:
Male C57BL/6 mice were divided into a control group, a 1-week T1DM group, and a 2-week T1DM group. PAI was used to measure the average sO2 (sO2 Avr) and average total hemoglobin concentration (HbT Avr). STI was employed to quantify the maximal longitudinal displacement and velocity of the intestine. Histopathological changes were evaluated via Chiu's score.
Results:
The results revealed that T1DM mice presented typical symptoms (e.g., weight loss and elevated fasting blood glucose) and progressive intestinal mucosal injury. sO2 Avr and intestinal motility increased first but then decreased with disease progression, whereas HbT Avr showed the opposite trend. sO2 Avr was strongly positively correlated with motility (r = 0.784 for displacement, r = 0.751 for velocity; both p < 0.001) and had high diagnostic efficacy for early intestinal injury (AUC = 0.914). Both PAI and STI demonstrated excellent diagnostic performance (AUC = 0.930 vs. 0.914, p = 0.817).
Conclusion:
PAI not only reveals a strong positive correlation between sO2 and the motility of the small intestine in DE, but also has good performance in evaluating early injury of the small intestine.
