We report a case of 21 years old male with Tetralogy of Fallot who underwent electrocardiogram-gated cardiac computed tomography angiography (CCTA) as part of preoperative evaluation. CCTA revealed a linear hypodense band within the ascending aorta, just proximal to origin of right brachiocephalic trunk, connecting the anterior and posterior walls. This case highlights the importance of CCTA in identifying such uncommon anatomical entities which can mimic aortic dissection on imaging and may predispose to unwarranted emergent surgical explorations, despite their indolent nature.
