Catheter-Based Renal Denervation in Acute Type B Aortic Dissection to Optimize Long-Term Pressure Control

Background:
Refractory hypertension (RH) is a critical complication of type B aortic dissection (TBAD) in acute and chronic phases. Adequate long-term pressure control reduces aortic-related adverse events, yet approximately 40% of patients do not achieve optimal therapeutic goals. Renal denervation (RDN) is an established technique used for chronic RH, but no experiences are reported in acute TBAD.
First-In-Human:
The current series presents the first-in-human experience of RDN in acute TBAD: 2 patients were treated with thoracic endovascular aortic repair because of RH. RDN was performed as an adjunctive procedure to optimize long-term pressure control. Both procedures were successful. At the 3-month follow-up, the patients experienced satisfactory blood pressure control with a reduced antihypertensive regimen.
Take-Home Messages:
RDN is a feasible adjunctive tool for achieving sustained blood pressure control in acute TBAD. Larger prospective studies are needed to confirm its safety and efficacy and its impact on long-term aortic outcomes.
