Tertiary Healthcare System01:21

Tertiary Healthcare System

Specialized care provided over an extended period is called tertiary care. Usually, a primary or secondary care physician will refer a patient to tertiary care. A patient's maximum physical and mental function is restored in tertiary care, which is caused due to the impact of a chronic illness or condition. Tertiary care aims to achieve the highest level of functioning possible while managing chronic illness. For example, a patient who falls and fractures their hip will need secondary care to...
Integrated Healthcare System01:20

Integrated Healthcare System

An integrated healthcare system (IHS) is a set of organizations that provides for or arranges to provide coordinated and continuous service to a defined population. The IHS takes responsibility for that particular population's health status and outcome, both clinically and fiscally. An integrated healthcare system is a well-organized, well-coordinated, and collaborative network. The integrated delivery system is a network that connects different healthcare providers to deliver organized,...

Issues And Trends In Healthcare Delivery System

The issues and trends in healthcare delivery are constantly changing. The COVID-19 pandemic is one recent issue that wreaked havoc on healthcare systems, causing a shortage of healthcare workers, high demand for medicines and supplies, and increased medical expenditure due to a lack of insurance. Other issues include rising healthcare costs and care fragmentation.
Cost Containment
Payment for healthcare services has historically promoted adoption of costly and often unnecessary or inefficient...

Secondary Healthcare System01:11

Secondary Healthcare System

Secondary healthcare is offered by a specialist, generally in hospitals or clinics for patients referred by primary healthcare providers. It occurs when a person has an illness or injury that requires specific medical care. Secondary care is often referred to as acute care. Secondary care can range from uncomplicated care to repair a minor laceration or treat a strep throat infection to more complicated emergent care, such as treating a head injury sustained in an automobile accident. Whatever...

Methods of Documentation VII: EMR

Electronic Medical Records (EMRs) primarily center around electronically documenting patients' health information within a single healthcare organization or practice. They contain essential clinical data related to a patient's medical history, diagnoses, medications, treatment plans, lab results, and other pertinent information relevant to the specific encounter or episode of care. EMRs are designed to streamline documentation and workflow processes within individual healthcare settings,...

Area of Science:

  • Health Informatics
  • Software Engineering
  • Cloud Computing

Background:

  • Legacy on-premise Electronic Health Record (EHR) systems in tertiary hospitals face challenges.
  • Modernization to cloud-native platforms is a strategic necessity.

Purpose of the Study:

  • Establish an optimized roadmap for transitioning monolithic EHR systems to a microservice architecture-based cloud-native EHR (MCEHR).
  • Address technical and strategic challenges in EHR migration.

Main Methods:

  • Conducted a 3-month strategic assessment and case study for a global healthcare provider.
  • Utilized semi-structured interviews with clinical informatics officers and system architects.
  • Performed phased hybrid migration of over 20 TB of Oracle-based legacy data to .NET 9 and RESTful APIs.
  • Developed a proof-of-concept (PoC) to simulate high-concurrency clinical workloads and ensure system resilience.

Main Results:

  • Achieved 100% transaction integrity during the transition to .NET 9 and MCEHR across 1,012 clinical test cases.
  • Identified migration of embedded business logic in legacy Oracle views as a key bottleneck.
  • Validated stable throughput and reduced risk of clinical workflow delays with RESTful API services in the PoC.

Conclusions:

  • Transitioning to MCEHR is complex but strategically vital for modern healthcare.
  • A staged roadmap involving core technology upgrades (.NET 9, RESTful APIs), selective component migration, and DevOps adoption is proposed.
  • The roadmap aims to mitigate risks and ensure successful EHR system modernization.
