Case:
A 2-year-old girl with congenital kyphoscoliosis and preoperative Cobb angle of 107°, presented with multiple congenital anomalies (T7-T8, L3-L4). She underwent a concave opening wedge osteotomy with convex posterior instrumentation using a tether cord (T8-L1 with allograft). At 42 months postoperatively, her coronal balance was improved.
Conclusion:
A posterolateral convex quadrant fusion using a tether cord and bone grafting with concave opening wedge osteotomy and rib-to-rib distraction achieved significant correction at the time of surgery. This is a viable option for skeletally immature patients with complex congenital deformities that preserves future surgical options.
