When Does Pigment Matter? Toward a Context-Aware Standard for Representation in Dermatology

Current calls to improve representation in dermatology often invoke "skin of color," diversity, or inclusion, but rarely specify when skin-tone variation is clinically essential versus when proportional documentation is sufficient. This ambiguity risks reducing representation to a generalized diversity metric rather than a clinically meaningful reporting standard. We propose a context-aware, three-tier triage model for dermatology research and educational resources based on skin tone or pigment phenotype. Pigment-critical domains are those in which skin tone directly alters diagnostic visibility, morphologic cues, dyspigmentation or scarring risk, or procedural safety, and therefore require intentional tonal breadth. Pigment-modulated domains are those in which skin tone influences sequelae, treatment response, or patient-reported outcomes, whereas the core phenotype remains recognizable across tones; these warrant proportional inclusion and tone-stratified reporting when clinically meaningful. Pigment-neutral domains are those in which skin tone has minimal relevance to the specific clinical question or endpoint; these may mirror local epidemiology while still reporting tone distribution for transparency. Aligning representation with pigment relevance offers a practical minimum standard for authors, reviewers, and journals.
