Healthcare Professionals' Perspectives on Technology in Transitional Care: Insights from Greece

Transitional care (TC) ensures continuity and safety as patients move between healthcare settings, yet technology use in these processes remains limited in Greece.

This study explored healthcare professionals' perspectives of how key technological determinants influence the adoption of digital health solutions in transitional care.

A cross-sectional survey was conducted among healthcare professionals involved in TC. Descriptive analyses examined how improvements in ICT literacy, interoperability, data strategy, and digital tools affect core TC processes.

Respondents reported a strong positive influence on technological improvements across all processes. The highest associations were observed between ICT literacy and follow-up coordination (0.72), and between interoperability and post-discharge support (0.68). Interoperability and data strategy showed the most consistent impact overall (mean > 0.6), emphasizing their role in improving communication and continuity of care.

User-friendliness should be a top priority when designing and implementing digital health interventions in TC while enhancing interoperability can strengthen collaborative decision-making.
