Molecular tumor boards (MTBs) are key instruments for enabling personalized precision treatment in oncology, leveraging patient's genomic and clinical data to inform treatment decisions. While they present a major advancement, particularly for patients with advanced diseases or unique molecular profiles, the preparation and execution of MTB case discussions remain highly resource-intensive. To combat this issue, we collaborated with participating clinicians to develop an extension for the MTB-cBioPortal, facilitating the preparation and presentation of digital slide sets directly within the platform. Based on an analysis of existing case presentations from different hospitals and tumor boards, we derived functional and data requirements for this feature. We leverage existing clinical and genomic information, along with visualization features, to offer tumor board-specific functionality, including mutation overviews and timelines covering the patient journey from initial diagnosis to the inclusion in the MTB. Additional components allow users to upload and incorporate (local) images and files, such as pathology slides and reports into a local storage component and integrate them into their case presentation. All resulting presentations are stored as FHIR resources using the existing MTB-cBioPortal infrastructure. The extension's usability was evaluated by a multidisciplinary cohort of MTB participants (n=7) consisting of pathologists, biologists and bioinformaticians using the System Usability Scale. It demonstrated excellent results charting in the 96th percentile and indicating strong potential for integration into routine molecular tumor board workflows.
