Handling of Nursing Care Information in Personal Health Records

Abstract:
The expansion of Personal Health Records (PHRs) in Japan has mainly focused on the integration of medical data such as clinical test results, diagnoses, and prescriptions, as well as collecting daily logs. However, with the rapid aging of the population and the diversification of care providers, there is a growing need to integrate nursing care information into PHRs to realize continuous, multi-professional care. This study reports the formative design and development of a web-based PHR application that enables the unified handling of both medical and nursing care information. The system allows users to record daily care routines, activities of daily living, home-visit records, and care-related observations alongside conventional medical data. The findings suggest that standardized care data elements within PHRs can be a foundation for holistic care in aging societies.
