Abstract:
This paper explores an alternative approach to documenting patient health data through the Problem-Oriented Medical Record, emphasizing the importance of understanding the evolution of health issues over time, particularly for polychronic patients. It proposes a dynamic model, the Health Issue Network (HIN), which utilizes Petri Nets to illustrate how clinical conditions evolve and interact. By integrating healthcare processes with clinical histories, the study highlights the need for a comprehensive framework that captures the complexities of patient care. The model aims to enhance communication among healthcare providers and improve treatment planning, particularly under the ISO 13940 standard (ContSys).
