Primary Lymphoid Organs01:16

Primary Lymphoid Organs

Primary lymphoid organs are pivotal in the formation, development, and maturation of lymphocytes, the white blood cells that serve as the backbone of our immune system. This crucial function underscores their fundamental role in maintaining our overall health and immunity. The two primary lymphoid organs of prime importance are the red bone marrow and the thymus.
The red bone marrow is a soft, spongy tissue nestled in the interior of long bones such as the humerus and femur. It is the site...

Secondary Lymphoid Organs

Secondary organs, including lymph nodes, the spleen, and mucosa-associated lymphoid tissue (MALT), work harmoniously to protect us from disease and infection.
The spleen is a vital organ in the lymphatic system, nestled in the upper left side of the abdomen. It is composed of two primary regions: the red pulp and the white pulp, each having distinct functions. The red pulp performs a significant role in blood filtration. It efficiently purges the blood of old or damaged red blood cells and...

Summary

This rare case highlights CD5-positive diffuse large B-cell lymphoma (DLBCL) and CD5-positive intravascular large B-cell lymphoma (IVLBCL) in the same patient. It suggests DLBCL may present with an IVLBCL-like pattern early on.

Area of Science:

  • Hematology
  • Oncology
  • Pathology

Background:

  • Diffuse large B-cell lymphoma (DLBCL) is an aggressive non-Hodgkin lymphoma.
  • Intravascular large B-cell lymphoma (IVLBCL) is a rare subtype characterized by neoplastic lymphocytes confined to blood vessels.
  • Co-occurrence of DLBCL NOS and IVLBCL in the same patient is exceptionally rare.

Purpose of the Study:

  • To report a unique case of a patient diagnosed with both CD5-positive DLBCL NOS and CD5-positive IVLBCL.
  • To investigate the potential early manifestation of DLBCL with an IVLBCL-like growth pattern.

Main Methods:

  • Histopathological review of biopsy specimens.
  • Immunohistochemical analysis for CD5 expression.
  • Retrospective analysis of clinical presentation and treatment history.

Main Results:

  • A 76-year-old male presented with lymphadenopathy and a prostatic mass.
  • Prostatic biopsy confirmed CD5-positive DLBCL NOS.
  • Retrospective review of a prior transurethral resection of the prostate (TURP) specimen revealed CD5-positive IVLBCL.

Conclusions:

  • This case demonstrates the simultaneous presence of DLBCL NOS and IVLBCL in different tissue samples from a single patient.
  • The findings suggest that DLBCL may present with an IVLBCL-like growth pattern in its early stages.
