[Odontoidectomy: a systematic review]

There is extensive experience of odontoidectomies for various lesions of craniovertebral junction worldwide. Various approaches including those through natural anatomical openings (transoral and transnasal), as well as transcervical one is used.
Objective:
To compare the outcomes of odontoidectomies through various approaches according to literature data.
Material And Methods:
A systematic review was conducted in accordance with the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines. We reviewed the PubMed, Medline and Google Scholar databases using the keyword «odontoidectomy».
Results:
Analysis included 128 studies describing treatment outcomes in 688 patients. Transoral approach was used in 446 patients, transnasal - in 192, transcervical approach - in 50 patients.
Conclusion:
Surgical approach for odontoidectomy significantly impacts surgery time, complication rates, initiation of oral feeding and length of hospital-stay.
