Global disparities in access to craniofacial surgery remain profound, particularly in low- and middle-income countries, where delayed or absent surgical intervention can lead to severe functional impairment, psychosocial distress, and preventable mortality. These inequities stem from a complex interplay of limited surgical infrastructure, workforce shortages, financial barriers, and inadequate public awareness. This article outlines a framework for ethical, sustainable, and effective collaboration in global craniofacial surgery. We first characterized the scope of surgical need in low- and middle-income countries and examined the systemic obstacles to timely, safe, and affordable care. Through a review of current initiatives and models of global surgical partnership, we identified key areas of opportunity: (1) investment in local training and retention of surgical providers; (2) integration of craniofacial care within the national health systems; (3) reduction of out-of-pocket costs through policy reform and philanthropic support; and (4) culturally tailored efforts to improve health literacy and early diagnosis. We emphasized the importance of bidirectional knowledge exchange, long-term partnerships, and the avoidance of vertical, short-term interventions that fail to build local capacity. Ethical global engagement must move beyond charity-based paradigms to embrace principles of mutual respect, capacity-building, and long-term partnerships. By adopting strategies that are context-sensitive and locally led, the global craniofacial surgery field can foster resilient systems of care and reduce the burden of untreated surgical conditions in underserved regions.
