Angiography of internal mammary artery (IMA) coronary bypass grafts from contralateral radial artery access can be technically difficult. In this case series, a simplified method for IMA graft angiography via contralateral access was performed utilizing a mother-and-child technique with a guide extension catheter advanced to the IMA ostium over a coronary guidewire. The study group consisted of 6 patients (5 male and 1 female) with ages from 53 to 81 years. IMA graft age ranged from 7 to 25 years. Angiography was successfully completed in all patients without complications or need for alternative access.
