Diagnostic insight into recurrent perianal abscess caused by Cutibacterium avidum: A case report

Abstract:
Cutibacterium avidum, a gram-positive anaerobic bacillus and skin commensal, is increasingly recognized as an opportunistic pathogen, though its association with perianal abscesses is uncommon. We describe a 53-year-old male with recurrent perianal abscesses unresponsive to repeated incision, drainage, and empiric antibiotics. Anaerobic cultures yielded small, opaque colonies identified as C. avidum by MALDI-TOF MS (VITEK® MS), confirmed on repeat isolation. Targeted therapy following surgical drainage resulted in uneventful recovery and no recurrence. This case highlights the pathogenic role of C. avidum in recurrent abscesses, emphasizing the importance of anaerobic culture and advanced identification methods in diagnosis and management.
