Summary

This study found no link between movement slowness and insomnia in essential tremor (ET) patients. Objective kinematic analysis did not correlate bradykinesia with sleep disturbances in essential tremor.

Background:

  • Essential tremor (ET) presents with heterogeneous motor and non-motor symptoms.
  • Movement slowness (bradykinesia) and insomnia are frequently reported in ET patients.
  • Potential shared mechanisms, like brainstem noradrenergic dysfunction, suggest a link between bradykinesia and insomnia.

Purpose of the Study:

  • To investigate the relationship between objectively measured movement abnormalities and sleep disturbances in essential tremor.
  • To assess if bradykinesia in ET is associated with insomnia severity.

Main Methods:

  • Kinematic analysis of finger-tapping was used to quantify bradykinesia in 29 ET patients.
  • Insomnia was assessed using the Insomnia Severity Index (ISI).
  • Patients were grouped based on the presence of bradykinesia and insomnia (ISI ≥ 8) for comparisons and correlational analyses.

Main Results:

  • 48.3% of ET patients showed bradykinesia, and 37.9% met criteria for insomnia.
  • No significant difference in insomnia prevalence was found between patients with and without bradykinesia.
  • No correlation was observed between kinematic measures of bradykinesia and ISI scores, though ISI correlated with tremor severity.

Conclusions:

  • Objective kinematic analysis indicates no direct relationship between movement abnormalities and sleep disturbances in essential tremor.
  • Bradykinesia in ET may involve cerebellar-basal ganglia circuits, while insomnia might stem from brainstem dysfunction.
  • Larger studies with neuroimaging and longitudinal designs are needed to fully understand ET's movement and sleep disturbances.
