Summary

Cardiac arrhythmias affect about one in four dengue patients, mainly as sinus rate issues. This highlights the need for ECG monitoring, especially in hospitalized adults and severe cases.

Background:

  • Cardiac involvement in dengue is increasingly recognized but poorly understood.
  • Evidence on the burden and types of arrhythmias in dengue is fragmented.

Purpose of the Study:

  • To systematically review and meta-analyze studies on cardiac arrhythmias in dengue patients.
  • To estimate the overall proportion of arrhythmias and describe major arrhythmia subtypes.

Main Methods:

  • Systematic search of multiple databases (PubMed, EMBASE, LILACS, etc.) up to November 2025.
  • Inclusion of observational studies reporting arrhythmia data in dengue patients.
  • Random-effects meta-analysis to pool proportions; subgroup analysis by age; risk of bias and certainty of evidence assessment.

Main Results:

  • Analysis of 35 studies with 6948 patients.
  • Pooled proportion of any arrhythmia was 24.48%, higher in adults (30.00%) than children (10.73%).
  • Sinus bradycardia (11.84%) and sinus tachycardia (10.63%) were most common; AV block was rare (1.33%). High heterogeneity observed.

Conclusions:

  • Approximately 25% of dengue patients experience cardiac arrhythmias, primarily sinus rate abnormalities.
  • Findings support baseline and risk-based ECG monitoring for dengue patients, particularly adults and those with severe disease.
