Background:

  • Dalbavancin, a lipoglycopeptide antibiotic, is increasingly used off-label for deep-seated infections.
  • Limited data exist on its use for suppressive antibiotic therapy (SAT).

Purpose of the Study:

  • To characterize real-world dalbavancin dosing strategies for SAT.
  • To assess the safety and efficacy of dalbavancin in SAT.

Main Methods:

  • A multicenter, multinational, retrospective descriptive study.
  • Included adult patients receiving dalbavancin SAT between January 2018 and March 2023.
  • Primary endpoint: description of dosing strategies; secondary endpoints: TEAEs and clinical failure.

Main Results:

  • Forty-six patients received dalbavancin SAT for a median of 201 days, often with retained devices.
  • The most frequent dosing was 1500 mg IV every 2 weeks.
  • 45.7% experienced TEAEs (most common: acute kidney injury), with rare discontinuation. 17.4% experienced clinical failure.

Conclusions:

  • This study represents the largest cohort of dalbavancin SAT to date.
  • Dalbavancin demonstrated feasibility and acceptable outcomes for SAT, with low rates of clinical failure and adverse event discontinuation.
  • Further research is needed to optimize dalbavancin dosing and evaluate long-term safety for SAT.
