A proposed clinical framework for intentional tooth replantation based on healing dynamics: case series

Abstract:
Intentional tooth replantation (ITR) serves as a definitive, tooth-preserving treatment but has limited success rate. Its clinical predictability is often hampered by inconsistent management protocols. Previously, ambiguity regarding the postoperative healing process has led to significant variability in follow-up schedules and assessment criteria. This article presents 4 successful cases of ITR with long-term follow-up, systematically describing the clinical and radiographic findings at postoperative intervals of 2 weeks, 1 month, 3 months, 6 months and beyond 1 year, 2 years or even 5 years. Based on these cases and our cumulative experience with 22 ITR cases. We systematically propose a clinical framework for the management of ITR based on the dynamic healing process, including follow-up timeline, key evaluation criteria, and clinical significance. The aim is to enhance the predictability and success rate of ITR, thereby facilitating the long-term preservation of the natural dentition.
