Association Between Obesity and Abdominal Aortic Aneurysm: Complex Interactions Between Waist Circumference and Body

Objective:
To investigate associations between general obesity (body mass index [BMI]) and central obesity (waist circumference [WC]) with abdominal aortic aneurysm (AAA) incidence and their interactions in a nationwide Korean cohort.
Patients And Methods:
This retrospective observational cohort study utilized the Korean National Health Insurance Service database. A total of 9,947,052 adults who underwent standardized health examinations from January 1, 2009, through December 31, 2010, were followed through December 31, 2019. Cox proportional hazards regression models assessed associations between obesity measures and AAA risk, adjusting for demographic, lifestyle, and metabolic factors. Interaction analyses evaluated combined effects of BMI and WC.
Results:
During a median follow-up of 9.3 years, the BMI-AAA relationship proved complex. While higher BMI was initially associated with increased AAA risk (unadjusted hazard ratio [HR] for BMI ≥30 kg/m2: 1.52, 95% CI: 1.43-1.61), after adjusting for waist circumference, this association reversed (HR: 0.89, 95% CI: 0.83-0.96). Conversely, WC demonstrated a consistent, independent relationship with AAA risk, with the highest category (≥100 cm in men, ≥95 cm in women) showing a 38% increased risk (HR: 1.38, 95% CI: 1.30-1.46) after comprehensive adjustment. Significant interactions between BMI and WC (P<.001) indicated non-additive combined effects.
Conclusion:
Central obesity, assessed by WC, shows stronger association with AAA risk compared with BMI, with significant interactions revealing complex interdependencies. These results support integrating both anthropometric measures into AAA screening guidelines and considering their interplay in risk assessment strategies.
