Purpose of the Study:

  • To synthesize and quantify the association between OSA and GV in individuals with diabetes.
  • To evaluate the effect of continuous positive airway pressure (CPAP) treatment on GV in OSA patients with diabetes.

Main Methods:

  • Systematic review and meta-analysis of published studies.
  • Searched seven databases from inception to March 2023, with an update in September 2025.
  • Included nine interventional and five cross-sectional studies; meta-analyses used a random-effects model.

Main Results:

  • CPAP treatment significantly reduced nocturnal standard deviation (SD) and marginally reduced nocturnal mean amplitude of glycemic excursions (MAGE) in single-arm studies.
  • Pooled estimates for 24-hour SD and MAGE were not significant.
  • Some measures like mean of daily difference showed significant reductions post-CPAP; findings for coefficient of variation were inconsistent. Cross-sectional studies suggested links between OSA severity and GV.

Conclusions:

  • Current evidence suggests OSA may influence glucose metabolism, particularly in type 2 diabetes.
  • Further randomized controlled trials (RCTs) and studies involving type 1 diabetes are necessary.
  • CPAP therapy may offer benefits for glycemic variability in specific contexts.
