This special article represents the 2025 installment in an annual series published by the Journal highlighting some of the most important new developments in the literature relevant to the practice of vascular anesthesia. Our review covers updates surrounding the use of prophylactic versus rescue subarachnoid drains for endovascular aortic repair, perioperative blood pressure management in patients with chronic hypertension, preoperative risk stratification and ischemic heart disease evaluation, and new guidelines on the management of descending thoracic and thoracoabdominal aortic pathologies. The authors thank the editors of the Journal of Cardiac and Vascular Anesthesia for the opportunity to contribute to this series.
