Unnecessary Surgeries in Modern Medicine: A Clinical and Ethical Challenge

Surgery is a cornerstone of modern medicine. Yet, when conducted without justifiable indication, it raises significant clinical, ethical, and financial concerns. Despite advances in evidence-based practice, unnecessary surgeries continue to occur.

This paper investigates the prevalence, underlying causes, and ethical implications of unnecessary surgical procedures globally, with emphasis on India, and proposes targeted reforms.

A narrative literature review was conducted using PubMed, Scopus, Google Scholar, and government health databases between 2000 and 2024. The search terms included "unnecessary surgery," "surgical ethics," "India," "cesarean rates," "spinal surgery overuse," and "legal cases medical negligence." The inclusion criteria encompassed peer-reviewed articles, WHO and ICMR reports, national health data, and court rulings in India. Selected literature was critically examined to identify key themes and data trends.

Global estimates suggest that over 11% of surgeries may be unnecessary. In India, cesarean section rates in private hospitals surpass 50%, far exceeding WHO recommendations. Contributing factors include financial incentives, outdated training, poor oversight, and over-reliance on imaging. Ethical concerns, including violation of patient autonomy and non-maleficence, are pervasive.

Addressing unnecessary surgeries requires a multi-tiered approach involving transparency, patient education, clinical reforms, and policy enforcement.
