Factors Affecting Illness01:18

Factors Affecting Illness

When a person's physical, emotional, intellectual, social development or spiritual functioning is compromised, this deviation from a healthy normal state is called illness. Illness creates stress that in turn harms individuals. Irritation, anger, denial, hopelessness, and fear are behavioral and emotional changes an individual experiences in the phases of illness. A variety of factors influence a person's health and well-being.
For instance, risk factors are connected to illness, disability,...

Summary

Frailty burden in ICUs did not significantly impact performance indicators like SAPS 3 and MPM0-III. This suggests frailty burden has limited relevance for ICU benchmarking and performance comparisons.

Area of Science:

  • Critical Care Medicine
  • Geriatric Medicine
  • Health Services Research

Background:

  • Frailty is common in ICU patients and linked to adverse outcomes.
  • Existing severity-of-illness scores (SOISs) do not incorporate frailty.
  • Investigating frailty's impact on ICU performance metrics is crucial.

Purpose of the Study:

  • To assess the influence of frailty burden on ICU performance indicators.
  • To evaluate the effectiveness of SAPS 3 and MPM0-III in the presence of varying frailty levels.
  • To determine if frailty burden affects standardized mortality rate (SMR) and standardized resource use (SRU).

Main Methods:

  • Retrospective cohort study utilizing prospectively collected data from 159 ICUs in Brazil and Uruguay.
  • Analysis of 242,141 patients, with frailty defined by a Modified Frailty Index (MFI) of 3 or higher.
  • Recalibration of SAPS 3 and MPM0-III models and reestimation of SRU parameters.

Main Results:

  • The median frailty burden was 23.0%, with a median hospital mortality of 14.8%.
  • No significant correlation was found between frailty burden and SMR or SRU for either SAPS 3 or MPM0-III.
  • Recalibrating models with MFI did not enhance performance estimates.

Conclusions:

  • Frailty burden demonstrated no significant impact on ICU benchmarking using SAPS 3 or MPM0-III.
  • These findings suggest limited relevance of frailty burden for performance comparisons in ICUs.
  • Further research may be needed to develop frailty-inclusive performance metrics.
