Background:

  • Preterm birth (PTB) is a significant public health concern.
  • The long-term cardiovascular health implications for women following PTB require further investigation.

Purpose of the Study:

  • To determine the cardiovascular mortality (CVM) risk in women with a history of spontaneous preterm birth (PTB).

Main Methods:

  • Population-based cohort study in the Netherlands.
  • Data linkage between National hospital Birth Registry and National Death Registry (1995-2017).
  • Analysis of CVM in women with any PTB and spontaneous PTB, with subgroup analysis for PTB < 32 weeks.

Main Results:

  • A cohort of 1,166,476 women was analyzed.
  • Women with spontaneous PTB had a higher CVM risk (aHR 1.38) compared to term birth.
  • Early spontaneous PTB (< 32 weeks gestation) showed the highest CVM risk (aHR 3.19).

Conclusions:

  • Women with spontaneous PTB have an elevated CVM risk, particularly those with early PTB (< 32 weeks).
  • This elevated risk may be linked to vascular mechanisms in a specific subgroup of spontaneous PTB.
