Introduction:
In the phase 3 DeLLphi-304 trial, tarlatamab demonstrated significant clinical benefit over second-line (2L) standard-of-care (SOC) chemotherapy options (topotecan, lurbinectedin, amrubicin) in patients with previously treated extensive-stage small cell lung cancer (ES-SCLC). However, the relative efficacy between tarlatamab and other available 2L regimens, such as platinum rechallenge or cyclophosphamide, doxorubicin, and vincristine (CAV), remains unassessed.
Materials And Methods:
A Bayesian network meta-analysis evaluated overall survival (OS), progression-free survival (PFS), overall response rate (ORR), and disease control rate (DCR) between tarlatamab and historical 2L ES-SCLC treatments. Analyses were conducted in the overall 2L population and platinum-sensitive subgroup (chemotherapy-free interval [CFI] ≥90 days).
Results:
In the overall 2L population, tarlatamab was associated with significantly longer OS versus all comparators (hazard ratio [HR] range: 0.50 [95% credible interval (CrI): 0.35, 0.70] to 0.62 [0.45, 0.87]), and favorable PFS (HR range: 0.68 [0.49, 0.93] to 0.94 [0.71, 1.25]). Tarlatamab also significantly improved ORR versus CAV, topotecan, irinotecan, and dinutuximab + irinotecan (odds ratio [OR] range: 2.58-3.76), and improved DCR versus CAV (OR: 2.35), while ORR/DCR differences with other comparators were not significant. In patients who were platinum-sensitive, tarlatamab similarly conferred a significant OS benefit over all comparators (HR range: 0.51 [0.31-0.84] to 0.63 [0.40-0.98]), including platinum rechallenge, although PFS, ORR, and DCR differences were not significant.
Discussion:
Tarlatamab demonstrated statistically significant and clinically meaningful improvement in OS compared with all currently available 2L treatments for ES-SCLC in the overall 2L population and platinum-sensitive subgroup, supporting its potential as a new SOC.
