Background:

  • Cerebral venous sinus stenting (CVSS) treats idiopathic intracranial hypertension (IIH) and pulsatile tinnitus (PT).
  • Unilateral stenting is common despite bilateral venous sinus stenosis.
  • Limited data exist on sequential contralateral stenting for persistent or recurrent symptoms.

Purpose of the Study:

  • To evaluate the safety and clinical outcomes of sequential contralateral CVSS.
  • To assess the efficacy in patients with incomplete or recurrent symptom improvement after unilateral stenting.

Main Methods:

  • Retrospective single-center case series (January 2019 - April 2025).
  • Included patients undergoing contralateral CVSS after initial unilateral stenting.
  • Assessed symptoms (headache, papilledema, visual field defects, PT) and composite scores; recorded procedural details and complications.

Main Results:

  • 16 of 118 patients (14%) underwent sequential contralateral CVSS; all were female, median age 30.5 years.
  • Papilledema resolved and did not recur; significant visual field defects resolved.
  • Composite symptom score improved significantly (p < 0.0001) from 7 to 1.
  • No intra- or post-procedural complications were reported.

Conclusions:

  • Sequential contralateral CVSS is safe and provides significant clinical benefit for selected patients.
  • This approach should be considered for persistent or recurrent symptoms post-unilateral stenting.
  • Prospective evaluation is warranted to further support this treatment strategy.
