[From paper records to electronic system change : clinical challenges]

The transition from paper records to digital forms is now well established, but changing from one electronic health record system to another is becoming increasingly common. These two stages involve distinct dynamics. The initial shift from paper to electronic records represents a structural transformation, with benefits emerging in the medium to long term, despite an initial increase in documentation burden and organizational adjustments. In contrast, transitions between electronic systems constitute a particularly vulnerable phase, associated with risks related to data migration, continuity of care, and patient safety. Successful transitions rely on robust clinical governance, process standardization, targeted training, and effective communication in both hospital and ambulatory care settings.
