Case Summary:
A 56-year-old woman with new-onset ankle swelling was concurrently found to have acute coronary syndrome and underwent percutaneous coronary intervention with drug-eluting stent placement to the left circumflex artery. Ongoing work-up revealed methicillin-susceptible Staphylococcus aureus bacteremia with associated mitral and tricuspid valve endocarditis and septic arthritis. While awaiting definitive surgical repair, she developed native and stent-associated mycotic aneurysms.
Discussion:
Mycotic aneurysms are serious complications of infective endocarditis, requiring surgical removal of the infected artery, augmented by bypass grafting or patch repair.

We hope to raise awareness of endocarditis-associated mycotic aneurysms in both native and stented coronary arteries and their potentially devastating course.
