  • A cross-sectional survey of 7004 rural residents (aged 18-70) across 6 Chinese provinces.

  • Physician-patient communication efficacy showed a significant negative indirect association (β=-0.026, P<.001), explaining 9.29% of the total effect.
  • Platform type significantly moderated the association: internet hospitals, professional platforms, and WeChat enhanced it, while short video apps and search engines weakened it.

Conclusions:

  • Online health information seeking directly improves patient adherence in rural populations.
  • This benefit is partially offset by reduced physician-patient communication efficacy.
  • Digital health strategies must consider platform-specific effects and promote health literacy and communication efficacy.
