Functional Limb Salvage After Landmine-Induced Hindfoot Injury Using a Composite Calcaneus-Achilles Allograft: A Case

Case:
A 22-year-old man soldier sustained a Gustilo-Anderson type IIIB hindfoot injury from a landmine blast. Initial limb salvage included polymethylmethacrylate spacer insertion, reverse sural flap, and external fixation. Definitive reconstruction used a composite calcaneus-Achilles tendon allograft and an anterolateral thigh flap with tensor fascia lata. By the 3-year follow-up, he regained full ambulatory function and independence in all daily living activities.
Conclusion:
Structural allograft reconstruction of the calcaneus and Achilles tendon can be a viable limb salvage option for blast injuries, facilitating functional recovery and independence when traditional approaches may lead to amputation or long-term disability.
