Recurrent dislocation of the peroneal tendon.

G Zoellner, W Clancy

    Summary

Recurrent peroneal tendon dislocation can cause ankle disability. A new surgical technique deepens the peroneal groove by reshaping the fibula, successfully treating ten patients without implants or tendon transfers.

    Area of Science:

    • Orthopedic Surgery
    • Sports Medicine
    • Anatomy

    Background:

    • Recurrent dislocation of the peroneal tendons is an infrequent cause of ankle disability.
    • This condition results from a shallow peroneal groove, predisposing tendons to subluxation.
    • Existing treatments may involve metallic fixation or tendon/ligament transfers, which carry risks.

    Purpose of the Study:

    • To present a simple surgical technique for correcting a shallow peroneal groove.
    • To address recurrent peroneal tendon dislocation without metallic fixation or soft tissue transfers.
    • To evaluate the efficacy of this novel surgical approach.

    Main Methods:

    • A surgical procedure was employed to deepen the peroneal groove.
    • The technique involves removing fibular bone to create a deeper groove.
    • A periosteal flap is preserved to maintain the tenosynovial channel integrity.

    Main Results:

    • The technique was successfully applied in ten cases of recurrent peroneal tendon dislocation.
    • No metallic fixation or tendon/ligament transfers were required.
    • The procedure effectively addressed the underlying deformity causing the dislocation.

    Conclusions:

    • This simple surgical technique effectively corrects peroneal groove shallowing.
    • It offers a viable alternative for managing recurrent peroneal tendon dislocation.
    • The method avoids complications associated with implants and soft tissue transfers.
