This is a case of a 55-year-old man with multivessel coronary artery disease and severe aortic stenosis. The patient presented with multiple recent hospitalizations for heart failure and had been turned down for heart transplantation approximately 1 year earlier. The heart team determined that the patient was high risk for sternotomy because of recent chest radiation and high risk for a transfemoral approach given his severe peripheral vascular disease. Ultimately, the patient underwent robotic minimally invasive direct coronary artery bypass with concomitant graft-assisted transapical transcatheter aortic valve replacement.
