Bone scintigraphy can reveal extraosseous radiotracer uptake that mimics significant pathology. We report a case of diffuse hepatic and renal cortical uptake on 99mTc-methylene diphosphonate bone scintigraphy after a recent intravenous iron infusion. Recognition of this imaging pitfall is crucial to prevent unnecessary investigations. Correlation with clinical and drug history is essential when interpreting unusual radiotracer biodistribution.
