[Oncocytoma of the eyelid: case series with histopathological correlation]

Eyelid oncocytomas are rare benign epithelial tumors that cannot be reliably distinguished clinically from other benign or malignant eyelid margin lesions. This case series describes three patients aged 64 to 77 years with slowly progressive, well-circumscribed, reddish-violaceous lesions of the lower eyelid, two of which were located peripunctally. Histopathology revealed sharply demarcated epithelial tumors composed of polygonal oncocytes with finely granular eosinophilic cytoplasm and predominantly adenoid or tubular architecture. Immunohistochemistry showed tumor cell positivity for cytokeratin 7 (CK7), a low Ki-67 proliferation index, and focal positivity of myoepithelial cell components for α-smooth muscle actin (SMA). After complete excision, no recurrence was observed during 10 to 18 months of follow-up. Oncocytoma should be considered in the differential diagnosis of chronic progressive, reddish-livid eyelid margin lesions, particularly in older patients. Histopathologic confirmation is essential, as complete excision is diagnostic and usually curative.
