Scar Remodeling With Adipose-derived Stem Cell Exosomes After Facial Deep Dermal Injury

Abstract:
Adipose-derived stem cell exosomes (ASC-exosomes) are emerging as a highly effective regenerative therapy for scar management. We present a case of a 30-year-old female patient who developed severe facial dermal injury and scar contracture following complications from a skin booster injection. Despite initial epithelialization, poor scar quality persisted. Treatment consisted of topical application of ASC-exosome with adjunctive microneedling to enhance intradermal delivery. During a 6-month period, therapy resulted in significant dermal healing, minimal scarring, and improved scar texture and pigmentation, without complications. Histologically, exosomes are known to promote tissue regeneration by modulating collagen remodeling, fibroblast differentiation, and angiogenesis. Our findings provide clinical evidence supporting the therapeutic potential of ASC-exosomes for scar remodeling after deep dermal injury, consistent with previous histological studies. Further research is needed to optimize delivery protocols and validate long-term efficacy.
