Neonatal Intensive Care Unit (NICU) discharge summaries are critical for patient care continuity.
Manual generation of these summaries is time-consuming and prone to errors.
Large Language Models (LLMs) offer potential for automating clinical documentation.

Purpose of the Study:

To develop and evaluate an edge-hosted LLM-assisted system for automated NICU discharge summary generation.
To assess the quality of AI-generated summaries against clinician-authored ones using a field-level evaluation framework.

Main Methods:

An edge-hosted orchestration pipeline (MORPHEUS) integrated bedside physiologic data and EMR data.
OpenAI's gpt-4o-mini API was used for language generation inference.
An LLM-based evaluator, guided by a clinician-defined rubric, assessed documentation quality across 72 fields and five dimensions.

Main Results:

AI-generated summaries showed superior performance across accuracy (0.93 vs. 0.75), completeness (0.91 vs. 0.67), actionability (0.93 vs. 0.72), coherence (0.94 vs. 0.74), and non-hallucination (0.95 vs. 0.78).
Error analysis revealed fewer omissions, unsupported assertions, and contradictions in AI-generated summaries.
Iterative prompt refinement led to improved quality and reduced omission rates.

Conclusions:

An edge-hosted LLM-assisted pipeline can produce NICU discharge summaries meeting or exceeding clinician quality standards.
The findings support the deployment of edge-orchestrated generative AI for critical neonatal clinical documentation.
A reproducible, clinician-grounded, field-level evaluation framework is effective for assessing AI-generated clinical documentation.
