    • ≥3 retrolisthesis levels (HR 5.50) and greater L4-S1 lordosis change (HR 1.09/degree) independently predicted revision.

    Conclusions:

    • Multilevel retrolisthesis burden, not isolated retrolisthesis, is associated with increased revision risk.
    • Combined multilevel retrolisthesis and significant lordosis change may indicate spinal vulnerability.
    • Findings are exploratory and require validation in larger studies.
