Background:
Regenerative orthopedic therapies are increasingly shifting from unimodal to multimodal concepts. Mechanical, biological, and electromagnetic stimuli are combined to activate regeneration and repair processes. Extracorporeal shockwave therapy (ESWT), platelet-rich plasma (PRP), hyaluronic acid (HA), and extracorporeal magnetotransduction therapy (EMTT) are particularly prevalent.
Objective:
This article analyses the biological foundations, mechanisms of action, and evidence from the past decades, with a focus on combined soft tissue engineering procedures incorporating shockwave therapy.
Results:
ESWT exerts angiogenic, anti-inflammatory effects via mechanotransduction and stimulates stem cells. PRP delivers growth factors and exosomal signals, while HA modulates the chondroprotective and viscoelastic microenvironment. EMTT expands the therapeutic arsenal through electromagnetic activation of cellular signalling pathways.
Conclusion:
Soft tissue engineering provides a biological foundation for multimodal treatment concepts within regenerative medicine, perioperative therapy, and inflammatory-degenerative soft tissue management.
