Extralaryngeal bifurcation of the recurrent laryngeal nerve (RLN) is common anatomical variation, but the multifurcation of the RLN with more than three branches is quite rare. Intraoperative neuromonitoring evaluating functional status of the nerve branches shows that the branched RLN may have functional variation. Identification and electrophysiological monitoring of the RLN can localize motor fibers within nerve branches and help preserve motor function.
