Effect of Intensive Outpatient Rehabilitation Based on the Schroth Method - A Case Report

Adolescent Idiopathic Scoliosis (AIS) is a three-dimensional spinal deformity of unknown etiology, defined by a Cobb angle >10°. Patients with large curves during growth are at high risk of progression and are often referred for surgery. Schroth-based physiotherapeutic scoliosis-specific exercises (PSSE) have shown potential to reduce progression and improve posture and respiratory function. This case report evaluates the effects of intensive Schroth rehabilitation combined with bracing on radiological and clinical outcomes in a high-risk patient. An 11-year-old girl presented with thoracic Cobb 62°, lumbar 38°, ATR 20°, and Risser 2. A 7-month intensive protocol was applied, including daily Schroth therapy (3 h/day) and full-time use of a 3D Schroth brace (20 h/day). Treatment aimed to improve trunk alignment, reduce asymmetry, and integrate corrective strategies into daily activities. Follow-up showed reduction of thoracic Cobb to 44° and lumbar to 28°, with ATR decreasing to 10°. Clinical improvements in posture and symmetry were also observed. These findings suggest that intensive Schroth-based rehabilitation combined with bracing may significantly improve outcomes and delay or reduce the need for surgical intervention in high-risk AIS.
