Effectiveness of Schroth Method and Brace in Correcting Adolescent Hyperkyphosis: A Case Report

Hyperkyphosis is a spinal deformity characterized by an excessive posterior curvature of the thoracic spine, most commonly developing during adolescence. If untreated, it may cause postural imbalance, pain, reduced spinal mobility, and functional limitations. Conservative treatment is essential during growth. The Schroth method uses three-dimensional exercises and breathing techniques to improve spinal alignment and postural control, while the Schroth brace supports corrective positioning and enhances therapy. This case study evaluates the effects of a one-year Schroth treatment combined with Schroth bracing on posture, spinal alignment, and respiratory outcomes in a patient with pronounced thoracic hyperkyphosis. An 11-year-old girl (Risser sign 1) with a kyphotic curvature of 71° completed 10 individual Schroth sessions followed by group exercises six times per week, alongside 20 hours of daily brace wear. The in-brace X-ray showed a reduction of 21° (from 71° to 50°). After one year without the brace, the curvature measured 55°. Sagittal imbalance improved from 76 mm to 35 mm. Significant improvements were recorded in VC, FEV1, and IC (t = 5.01, t = 4.1, and t = 2.86, respectively; p < 0.05). The results support the use of combined Schroth therapy and bracing as a conservative approach in the management of adolescent thoracic hyperkyphosis.
