Abstract:
Swyer syndrome (46,XY complete gonadal dysgenesis) is a rare disorder of sex development characterized by a female phenotype in the absence of functional gonads. Despite ovarian failure, pregnancy can be achieved through in vitro fertilization (IVF) using donor oocytes in patients with a preserved uterus. This report describes the case of a 29-year-old woman with Swyer syndrome and prior bilateral adnexectomy who underwent IVF with donor oocytes. Following endometrial preparation in an artificial cycle, a single frozen embryo transfer was performed. The first implantation resulted in an interstitial ectopic pregnancy, which was successfully treated with methotrexate. Six months later, a subsequent IVF cycle resulted in a normally progressing intrauterine pregnancy. The pregnancy was uncomplicated and ended in the delivery of a healthy male neonate at 39 weeks of gestation by cesarean section. This case highlights both the reproductive potential of patients with Swyer syndrome using assisted reproductive technologies and suggests that early transvaginal ultrasound should be mandatory in all IVF pregnancies, regardless of adnexal status, as ectopic implantation may occur even after bilateral adnexectomy.
